Instructions for Submitting UGAA Forms Online

  1. Enter your login information.
  2. Enter your name, ID, email address, major, degree, and minor/specialization if applicable.
  3. Select the form you would like to submit:

Academic Program AUDIT REQUEST

An academic audit provides a summary of a student's academic status to date and lists the courses to be completed in order to receive a degree. Students can request an official academic audit from the Office of Undergraduate Academic Affairs after they have earned a minimum of 60 semester hours  (90 semester hours for architecture students).

Change of major

Select the new major and enter a reason for changing. Upon submission, a copy will be sent to the appropriate department for review. If approved, a copy of the approval and information on the student's new advisor will be sent to the student and to UGAA so their record can be updated.

CHANGE OF STATUS

Students who wish to change from non-degree-seeking to degree-seeking must complete this form no later than two weeks prior to registration, and must have at least one semester (12 hours) of relevant coursework at IIT and be in academic good standing.

Enter your intended major and the semester you will become degree-seeking.

Declaration of double major

Students majoring in Applied Analytics, Applied Mathematics, Computer Information Systems, Global Studies, Humanities, and Psychological Science are eligible to declare a Double Major. Upon graduating, the student will receive one diploma with both majors listed.

Select your second major and enter a reason. Upon submission, a copy will be sent to the appropriate department for review. If approved, a copy of the approval and information on the student's new advisor will be sent to the student and to UGAA so their record can be updated.

declaration of dual degree

Students who wish to earn two undergraduate degrees can declare a Dual Degree. Students working towards a Dual Degree must complete 15 hours above and beyond those required for their first degree. Upon graduating, the student will receive two diplomas.
