BCBS Ancillary (Formerly Dearborn National) Life Insurance
IIT provides at no cost a term life insurance benefit of two times annual salary to a maximum of $500,000 through BCBS Ancillary (formerly Dearborn National) Life Insurance. An accidental death and dismemberment (AD&D) policy provides additional coverage in the same amount. At age 65 the coverage level is reduced, as shown in the table below:

Fort Dearborn Life (FDL) reviews all enrollment forms before an employee is approved for the plan, and occasionally will require additional information before approving an enrollee. Fort Dearborn will provide details as to what is needed (visit from an FDL doctor, blood tests, etc).
